I just fell in love with this font. Any ideas?

It is a custom version of PTL Notes from Ole Schäfer. See this (german) Fontblog post.

That IS customized! The tops and bottoms of the S's curve inwards in the custom version and look better. Now that I look at it, a lot of it has been tweaked. But the basic structure is there!
