Address with forced italics?

Hello everyone, can anyone ID this address? The italics look forced to me but I can't seem to find an exact match for the roman or the italic, thanks guys.

Adobe Serif MM, which means whoever made the PDF this originally came from didn't embed the font properly or convert the text to outlines first.
